Formulate two actual nursing diagnoses and one risk for diagnosis
Myles is a 20-year-old man who was brought to the emergency room by the campus police of the college from which he had been suspended several months ago. A professor had called and reported that Myles walked into his classroom yelling and accusing him of taking his tuition money.
Although Myles had much academic success as a teenager, his behavior has become increasingly odd during the past year. He quit seeing his friends and no longer seems to care about his appearance or social pursuits. He began wearing the same clothes each day and seldom bathed. He lives with several family members but rarely speaks to any of them. When he does talk to them, Myles would say that he found clues that his college was just a front for an organized crime operation. His sister said that she has often seen him mumbling quietly to himself and at times, he seems to be talking to people who are not there. He would emerge from his room and ask his family to be quiet even when they are not making any noise.
Myles began talking about organized crime so often that his father and sister brought him to the emergency room. On examination, Myles is found to be a poorly groomed young man who seems inattentive and preoccupied. His family said that they had never known him to use drugs or alcohol, and his drug screening results are negative.
Myles is now on the behavioral unit under voluntary admission. This is his first psychiatric hospitalization. He is diagnosed with paranoid schizophrenia.
Based on the case study:

2. Prioritize the nursing diagnoses that you have formulated.
3. For the #1 priority problem, identify three client goals/outcomes (must meet the SMART criteria).
4. Identify five nursing interventions for the achievement of client goals/outcomes.
